Adobe Acrobat FileAgreement for Use of Facility
St. James Presbyterian Church provides the use of its facilities and services to non-profit, educational and/or charitable organizations, if properly scheduled and approved by the Church. Presbyterian Church-related organizations may use the building without charge. All other organizations must return this document for approval by the Church, along with a check for the scheduled fees and must agree to abide by its terms.

This form is for church members wishing to request reservation of church facilities. If your group is not represented by a member of Saint James, please use the Agreement for Use of Facility form.

Use this form if you would like to be reimbursed for church-related expenses. Receipts must accompany all requests. Please avoid purchasing personal items on the same receipt.

Parents and guardians should use this medical release and permission form when their son or daughter participates in Saint James Presbyterian Church youth activities.
